South Bend – Holly Lynn Tate, 39, passed away unexpectedly on Saturday, September 26, 2026, at Memorial Hospital in South Bend, Indiana, with her loving family by her side.
Holly was born on September 30, 1986, in Dearborn, Michigan, to Randy and Keri (Ucinski) Tate. Her family later moved to Plymouth, Indiana, where she grew up. She was preceded in death by her father, Randy Tate.
Holly met David Ousnamer, whom she lovingly considered the love of her life. The two were married on July 30, 2005, at 7:30 p.m. in Plymouth, beginning a life together filled with love, family, and memories.
Those left to cherish Holly's memory include her beloved husband, David Ousnamer; her children, Gillian (Jeffrey) Lewis, Elizabeth Darlene Ousnamer, and Scott Buckingham Jr.; her grandchild Haley Sage Isabella Lewis; her brother, Randy Roy Tate Jr.; her mother, Keri (Joe) Mersch; her grandparents, Jean and Clyde Smith; her mother- and father-in-law, Nancy (Bob) Bechtel; and David's father Craig Ousnamer along with many beloved nieces and nephews, extended family members, and numerous friends and associates.
Holly had a love for excitement and adventure. She especially enjoyed attending Night of Destruction races at the local racetrack, VR gaming, riding motorcycles, traveling, fishing, and making special trips to Myrtle Beach. Always eager to embrace new challenges, she was proud to have recently earned her Class A CDL and motorcycle license.
Known for her boundless energy and outgoing personality, Holly had a special social aura that drew people to her. She had a remarkable ability to brighten even the darkest of days and make the most ordinary, everyday tasks feel like an adventure. Holly had overcome hardships in her past that might have broken another person's spirit, but she continued forward with determination, strength, and a zest for life. Above all, she was a loving wife, mother, daughter, and grandmother whose warmth, laughter, and unforgettable personality will live on in the hearts of all who knew her.
The family will receive friends on Monday, October 5, 2026, from 4:00 until 7:00 p.m. at Chapel Hill Funeral Home, Osceola, Indiana. No other services are planned.
